Customer Satisfaction Surveys
A total of 66 client exit surveys and 11 referring agency exit and family
exit surveys were completed during the July to September 2011 quarter.

Group Care Intensive
Services: Five client surveys were completed and five
referring agency and family surveys
were completed.

Exit surveys indicate that 80%
of clients felt that the program helped them to deal with their problems, while
20% of clients had no opinion.
o
Exit surveys indicate that 100%
of referring agencies and families surveyed agreed that they would recommend or
use the program again.
o
The average overall rating on a scale of 1-5 (with 5 being the highest)
on the referring agency and family exit surveys was a 5.

Group Care Independent
Living: Seven client surveys were completed and
five referring agency and family
surveys were completed.

of clients surveyed felt that the program helped them to deal with their
problems.

of referring agencies and families surveyed would recommend or use the program

the clients surveyed felt that the program helped them to change their
attitudes. 77% of clients surveyed from the STEAR groups felt that the groups
helped to improve their relationships, while 14% of clients had no opinion, and 9% of clients answered not applicable. 98%
of clients surveyed agreed that they would recommend the program to other
people, while 2% of the clients
surveyed had no opinion.

program again.

highest) on the referring agency satisfaction surveys was a 9.
Post-Discharge Outcomes
A total of 105 post discharge outcomes were
obtained during the July to September 2011 quarter.

Group Care Intensive Services: Of the 39 surveys attempted and distributed from
July to September, 30 surveys were completed.
o
Follow-up surveys indicate that 17%
of the clients surveyed were discharged due to completion (graduation) of the
program, while 100% of these clients
were discharged according to their plans of care. Of these 30 former clients, 63% of them are currently maintaining
in a less restrictive environment and 53%
are attending an educational setting, with 70%
free of any criminal activity.

Group Care Independent Living: Of the 17 surveys attempted and distributed
from July to September, 12 surveys were completed.
o
Follow-up surveys reveal that 0%
of the clients surveyed were discharged due to completion (graduation) of the
program, but 17% of the clients
surveyed were discharged according to their plans of care. 75%
of the discharged clients are currently maintaining in a less restrictive
environment, 50% are currently in
school/vocational training or gainfully employed and 75% are free of criminal activity.

July to September, 62 surveys were completed. Forty-seven of these surveys came from
clients who participated in the domestic violence (S.T.E.A.R.) groups, and fifteen of these surveys came from
clients who participated in the anger management (P.A.C.E.) groups.

Follow-up surveys from both STEAR AND PACE groups indicate that 94% of the clients surveyed are no
longer participating in therapy since their final sessions at FCC, and 98% of clients are not involved with
any other support services. 94% of clients surveyed indicate that
they are utilizing the skills that they learned from therapy in their daily
lives and 100% of the clients
surveyed have not been involved in any criminal conduct. Out of the 47 STEAR surveys completed and returned, 83% of clients indicate that they are still living at home with
their spouse or partner, while 83%
report that their relationships have improved because of their participation in
the groups.

Rehabilitative Behavioral
Health Services (RBHS): Of the one survey
attempted and distributed from July to September, one survey was
completed.
o
Follow-up surveys indicate that 100%
of the clients surveyed had planned discharges, and 100% of clients still reside in a less restrictive setting with
their family or stable caregiver. 100% of clients surveyed indicate that
they are in public school, vocational training, have obtained their GED, or are
gainfully employed. 100% of the clients surveyed have not been involved in any criminal
conduct.
